Please use this form to list the domain(s) where you plan to test Chrome as an independent app and to indicate a point of contact + email for communications around this Trusted Tester program. 

You will receive detailed follow-up information (how to enable, what to test, etc.) as soon as your test domain is granted access to the admin controls for Chrome as an independent app.
